How to use in Google Forms

  1. Go to forms.google.com and click the blank form (+) or open Template gallery → Work.
  2. Add a Linear scale question: "How satisfied were you with your experience?" Set scale 1–5 with labels.
  3. Add Short answer for product or service name, then two Paragraph questions for positive and negative feedback.
  4. Add a Multiple choice yes/no question for recommendation intent.
  5. Under Settings → Responses, link to Google Sheets. Toggle email collection only if you need follow-up contact.
  6. Click Send to share the link via email, embed on your site, or post in a receipt message.

Yes. In Google Forms settings, uncheck "Collect email addresses" and do not add a name field if you want fully anonymous responses.
