How to use in Google Forms

  1. Open forms.google.com and create a blank form. Title it with your event name and date.
  2. Add Short answer fields for name and email. Mark both as required.
  3. Add Multiple choice: "Will you attend?" with Yes, No, and Maybe options.
  4. Add Short answer for guest count and Paragraph for dietary needs.
  5. Optional: use Go to section based on answer so "No" respondents skip guest-count questions.
  6. Enable Limit to 1 response in Settings if attendees have Google accounts and you want one RSVP per person.
