How to use in Google Forms

  1. Create a new form at forms.google.com.
  2. Click Settings → Make this a quiz. Choose when to release grades.
  3. Add a Short answer question for student name (or rely on Google account auto-collection).
  4. Add Multiple choice questions. Click Answer key on each to mark the correct option and set points.
  5. For "select all" items, use Checkboxes with partial credit if needed.
  6. Share via Google Classroom assignment or copy the form link for in-class use.
